Need a sample Doctor’s letter for Federal IDs?
Click on button below to download the sample letter for gender updates to Passports or Social Security Cards. This letter is from the National Center for Trans Equality ( transequality.org) and should be able to be used for both Passport and Social Security updates. If you get additional requests for information or documents from the Passport agency, submit the documents within 90 days and feel free to contact TARP or the NCTE for more help. If Social Security denies this form letter, get in touch for help!

Each County’s page has fill-able court forms, example documents and thorough instructions packets for filing for name changes, gender change recognition orders and out of state birth certificate update orders. Be sure to download the Name and Gender Change in Court Instructions packet- it will walk you through which of the rest of the forms you need and how to complete them. There is also a button for Fee Waiver packets too if you want to apply for a fee waiver. These forms are for adults only- minor forms will be added soon! Be sure to download both the Petition and the Proposed Order forms for your gender recognition court processes. Your Name Change Petition doesn’t need a Proposed Order.
