Adult Gender Change Recognition Forms with a Birth Certificate Update

Download these forms if you are adult who needs to update your out of state birth certificate through filing for a Gender Change Recognition Petition in Court. You will need the first form, which is the Petition, as well as the next form link which is the Proposed Order. There is also an example completed petition linked as well. This form DOES include what is needed for updating out of state birth certificates. Reference Section 3 and 6 of the Court Instructions Packet for directions.

Please be aware that due to the confusing intersection of laws between states and the uncommon nature of these types of proceedings, some Judges may not feel they have the authority to grant these kind of birth certificate update orders. TARP is working to craft a legal brief signed onto by community organizations that can be submitted alongside any birth certificate update petitions that will hopefully help make these processes more predictable. If you are wanting to petition for one of these types of court orders, please email the TARP coordinator at dusty@lavenderrightsproject.org to get access to the brief and specialized arguments if they aren’t yet up on the document bank.

How to Apply for A Fee Waiver

If you want to apply for a fee waiver to ask the Court to waive or reduce your filing fee cost due to being low income, you should review this packet and complete the included forms from Washington Law Help. These fee waiver forms are to be turned in when you file for your name/gender change. Remember that Ingersoll Gender Center can help with the costs of filing fees for trans related court processes.
